Diablo 4’s Lead Class Designer confirmed on Twitter that these items can be acquired by defeating level 85 or higher enemies. He also stated that “You get them anywhere you can get a regular Unique,” which means there’s a chance to acquire them in WT3: Nightmare and WT4: Torment.
The possibility of getting one of the items by doing these activities is one in a million, so don’t hold your breath. Apparently only 142 players have gotten their hands on these items, which only proves how rare they are, but there’s no harm in trying, so give these activities a shot.
