In Episode 11, titled ‘Curse-Revealing Chapter, Part 5,’ another tragedy unfolded in Hinamizawa when numerous murders happened in the town on a single day.
Earlier in the episode, Satoko tricked Keiichi into coming to the Hojo house and killed her with the same bat she killed her uncle with.
To add to it, she manipulated Ooishi and lied that Rika used Keiichi as a pawn. Under the effect of the curse, Ooishi went to the shrine and killed Rika and many others.
In the end, Satoko shot Ooishi when he realized who the real culprit was. Next, she shot herself in the head after making sure Rika is dead while leaving Rena alive.

Episode 12 Preview & Speculation
It’s confirmed that Keiichi didn’t actually bash Teppei’s head with the bat in Episode 13 of the last season. It was Satoko who did all this, and it looks like the red-tinted scene is actually Satoko’s own retelling of what happened.
Furthermore, because of the memories from past fragments, Rena’s memory of Satoko killing Ooishi was possibly triggered when Satoko pulls out the gun. This is where the episode ended on the cliffhanger.
Since Rena is the only alive witness of whatever happened at the shrine, it seems like leaving her alive in that world is going to backfire in some way or other.
The upcoming episode will mark the onset of the Nekodamashi arc, which will be another part of the answer arc. I’m curious as to what new developments it has in store for us.

Episode 12 of the Higurashi: When They Cry – Sotsu (2021) anime, titled “God-Entertaining Chapter, Part 1,”, has been released on Thursday, Sep 09, 2021.
Since this anime runs on a weekly schedule, new episode releases are seven days apart.

Episode 11 Recap
After Satoko kills her uncle with a bat, she goes straight into the bathroom to wash off the blood. Just when she comes out of the shower and changes her clothes, Rika comes outside. The two head to the Watanagashi festival together.
While everyone watches Rika’s traditional dance during the festival, Satoko grabs Keiichi’s hand and takes him outside the shrine. She thanks him for how he fought for her and asks if she can call him Nii-Nii. She then asks him to come home as she has something to give him.
Meanwhile, Ooishi keeps an eye on Hojo’s house to catch the culprit behind Oyassan’s disappearance and therefore sees Satoko taking Keiichi inside. Keiichi steps inside the house, and while he tries to find the switch, Satoko grabs the bat.
Just when he turns on the light and sees blood around him, Satoko hits him hard on the head. Blood splashes down his head, and he collapses. Next, she screams out loud on top of her lungs. Ooishi rushes inside the house and finds Satoko covered in blood.
He sees Teppei and Keiichi lying down in the room. While crying, Satoko blames it all on Keiichi and lies that Teppei and Keiichi hit each other to death. Ooishi couldn’t believe Keiichi did it by himself and suspects someone else used him as a pawn.
Satoko lies again and says she heard Keiichi saying that Rika told him to do this. Under the effect of the Hinamizawa curse, Ooishi picks up the bat and runs towards the Oyashiro-sama shrine. At the shrine, everyone searches for Satoko and Keiichi.
Suddenly they see Ooishi approaching Rika with blood all over his clothes and a bat in his hand. He chokes Rika and shoots everyone who tries to save her while asking her to reveal the secrets of the curse. Just when he kills Rika, Satoko also reaches there.
Ooishi realizes the real culprit behind all the tragedy and points his gun towards her, but Satoko takes out her gun and shoots him in the head. She then shoots Rika to make sure she is dead, followed by shooting herself.
