Kaori Miyazono died in Your Lie in April after undergoing a failed surgery to cure Friedreich’s ataxia. She often frequented the hospital, resulting in a less-than-desirable lifestyle. In the end, Kaori decided to undergo a surgery that ultimately resulted in her death at the age of fourteen.

While the name of the illness that inflicted Kaori was never revealed, from the symptoms she showed, it is assumed to be Friedreich’s ataxia, which is a genetic disease that causes difficulty walking due to a loss of sensation in arms and legs.

Furthermore, as the disease progresses, the patient loses their sight and hearing. Nevertheless, people suffering from it are still bound to reach the age of 60.

Suppose Kaori did indeed suffer from Friedreich’s ataxia, as seen by her difficulty to walk. In that case, her death is ever more heart-breaking as it was entirely possible for her to live longer if she had not undergone surgery.

However, throughout the series, Kaori is shown to be lively and free-spirited, a trait that is heavily reflected in her music. She feels through music, and it is something that kept her going through her hard times.

If Kaori did not undergo surgery and continued living, she could lose the senses that allowed her to play, which would have been worse than death for her.

Your lie in April is adapted from the manga of the same name by Naoshi Arakawa. Its plot revolves around a piano prodigy Kousei Arima. But he lost his playing ability on his mother’s sudden death.
